Printed on the front is the NIN logo with NINE INCH NAILS printed on top of the logo.
The back of the t-shirt has the US Catalog number with NIN Logo and Halo Eleven printed towards the top. The layout of this print is in line with previous CD releases but is not present on The Perfect Drug CD itself.
